General Summary: The Day Services Coordinator supports the current day site. This position provides monitoring and support to those in day services to ensure the highest level of quality services possible, while maintaining compliance with the Individual Service Program- ISP. The Day service Coordinator is responsible for overseeing the Day Service program or programs with a broad range in scope Day Services Coordinators will possess technical and professional knowledge, skills, and abilities relating to the assignment as well as leadership skills necessary to administer the program.

Salary Range: $19 to $22 per hour
Qualifications
Educational and Experience Requirement:
- An associate degree in a health, social service, business or related field and 3 years of work experience in program administration, development, evaluation, and /Or contract administration; OR
- Five (5) years’ experience in program administration, development, evaluation and /Or contract administration.
Knowledge Of
- Performing and coordinating intake of new clients.
- Development of treatment plans by conducting behavioral and skill assessment of clients though behavioral assessments to identify and access individual strengths and needs.
- Function as a member of a treatment team for adults and. This includes working with a therapist, guardians, physicians, DHHS workers, and child and family services professionals.
- Work in collaboration with schools, providers, mental health professionals, and other community resources.
- Completing monitoring activities and providing technical assistance to ensure the accurate and timely completion of required documentation.
- When on call, provide timely responses for off hours crisis management.
- Exception funding - will be reviewed by our QA/QI before submission.
- Program writing - will be reviewed by our QA/QI before implementation.
- MGR - will be reviewed by our QA/QI for accuracy.
- Intake collection/meeting - will be reviewed by our QA/QI for accuracy.
- Other tasks assigned by the administration team if applicable.
- Email and phone messages
- Therap: Ensure contractors are completing daily data (MAR, Side effect log, ISP Programs, T-Logs, S Comms, GER, and RRC Forms. The staff should never be more than 2 hours behind or 2 hours for the MAR.
- Read T-logs
- Review, Edit and Save GERs
- Attend the meeting
- Work along with finance for any needs of the individual or for authorizations
- Work along with HR and training to assist in communications with background checks or training requirements
- Review any SComms in Therap
- Thursday morning team meeting at 12 pm in the conference room
- Data Validation (Checking to ensure data is completed by staff)
- Complete RRC forms and submit to committee by due date assigned
- Complete and audit of client’s progress in their program (MGR)
Prepare for any ISP meetings
Yearly Duties
- Attend and prepare for all ISP meetings.
